Visiting Angels Offers One on One Comfort Care with Our Ready-Set-Go Home Program in Augusta, Evans, Martinez, and Surrounding Areas

At Visiting Angels of Augusta, we have designed a transitional care program to help seniors transition successfully from hospital care to home care. Known as the Ready-Set-Go Home program, it is a three-prong approach to reduce hospital readmission – a serious risk for elderly individuals in the 30 days after a hospital discharge.

Approximately 20% of seniors on Medicare who are discharged from the hospital return to the emergency room within 30 days. According to these numbers, older adults have a lower chance of recovery if they are re-hospitalized in the month following a discharge.

Three-Prong Approach to Reducing Hospital Readmission

We use a three-prong approach for our Ready-Set-Go Home program to reduce hospital readmission:

  • At the Hospital:. We support your loved one and family members before discharge from the hospital with on-site assistance. Our comfort care providers can also guide families through the discharge process and provide valuable information. It includes questions you can ask nurses, doctors, and hospital staff to better prepare for the transition as well as helpful tips on reducing the risk of readmission.
  • At the Client's Home: Once at home, our comfort care providers ensure compassionate companionship and assistance, ensuring your loved one is safe during the recovery process. Should our caregivers notice any changes with your senior’s cognitive or physical health, they will notify you right away. Also, our aides offer unpaid family caregivers the opportunity to rest with Respite Care
  • At the Office: Our office staff works diligently behind the scenes to provide quality home health care, utilizing tools and programs that flag risks to reduce hospital readmission. Additionally, they provide reminders for appointments and medication, offering accompaniment to medical appointments, and can coordinate delivery for food, equipment, and medication.
