Life Care Navigation covers a wide range of services from basic programs such as companion care and personal care to specialized services, including our Safe & Steady Fall Prevention Program. Your loved one's initial care plan can include as few or as many of these services as needed.
Over time, we customize this care plan by adjusting the existing services or adding new ones. As a result, your loved one will have a personalized care plan every step of the way.
